Home Diabetes Management Service
The Home Diabetes Management service is provided internally within Active Ageing and Community Care, where diabetic homebound clients who are lost to follow-up due to their limitations are assessed in their own homes.
Aim:
The Home Diabetes Management service is a targeted, community-based initiative designed to support older adults living with diabetes, particularly those who are homebound. Diabetic clients already receiving services from the Active Ageing and Community Care (AACC) will be identified by AACC professionals and referred to the Home Diabetes Management Team.
What you’ll get:
Once a client is referred, the case is discussed, and a healthcare plan is tailored. The AACC’s General Practitioner and the nurse will conduct a domiciliary visit offering a comprehensive, in-home assessment and ongoing support to enhance diabetes management and promote safe, independent living. Other professionals from the multidisciplinary team may be involved, and follow-up visits are set according to the client’s needs.
Eligibility:
- Clients of the Active Ageing and Community Care;
- Clients diagnosed with Diabetes Mellitus who are homebound and therefore are unable to leave their homes without assistance;
- Clients who are not being followed up in relation to their diabetes.
How to apply:
- The Commcare and/or other AACC professionals will identify eligible clients and refer them to the Home Diabetes Management Team.
