Community General Practitioner Service

The Community GP Service aims to provide a holistic approach to the management of chronic disease to elderly clients who are homebound due to physical or mental health problems. Clients already receiving services from the Active Ageing and Community Care (AACC) and suffering from complex medical conditions will be identified by AACC professionals and referred to the Community GP.

WHAT YOU’LL GET

Once a client is referred, the case is discussed and a healthcare plan is tailored, the Community GP will conduct a domiciliary visit with the AACC professional when necessary. Other professionals from the multidisciplinary team may be involved and follow-up visits are set according to the client’s needs.

ELIGIBILITY

  • Clients of the Active Ageing and Community Care; Clients who are homebound and therefore are unable to leave their homes without assistance;
  • Clients with complex medical issues.

How to Apply

  • The Commcare and/or other AACC professionals will identify eligible clients and refer them to the Community GP.
