Dementia Intervention Team

The Dementia Intervention Team (DIT) provides a holistic community-based support service for persons with dementia and their caregivers. A holistic assessment of the caregivers’ and care recipients’ situation, is carried out in the home, where the person with dementia is residing. Following an assessment, a care plan is tailor made for each case – which might include education and training; support, and advice on safety and coping strategies for both the person with dementia and their caregivers. Ultimately, the main aim of the team of professionals is to enable people with dementia to continue actively ageing in the community and improve their quality of life.

WHAT YOU'LL GET

The team of professionals provides a specialised care plan to support the person with dementia and his caregivers. Periodically the team will follow-up on each case and the care plan adapts according to the person’s needs. The team becomes a reference point for support to those persons with dementia and their family.

ELIGIBILITY

General eligibility criteria:

  • An applicant needs to be diagnosed with a type of dementia
  • The applicant needs to be living in the community
