Community Geriatrician Service

The Community Geriatrician Service aims to provide a comprehensive geriatric assessment to older persons who are homebound due to physical or mental health problems. The Community Geriatrician will carry out a domiciliary medical visit together with professionals from the CommCare Assessment Unit and the Dementia Intervention Team.

WHAT YOU'LL GET

Following receipt of the application and referral, the case will be discussed between the Community Geriatrician, CommCare, the Dementia Intervention  team and the applicant’s General Practitioner. After such a meeting, a home visit would normally follow, particularly when dealing with complex medical issues requiring specialist geriatric management. Other professionals from the CommCare multidisciplinary team may also be involved, with follow-up visits according to the client’s needs.

ELIGIBILITY

General eligibility criteria:

  • Persons over 60 years of age;
  • Living in the community;
  • In need of a non-acute intervention;
  • Unable to go out of home and attend a geriatric outpatient clinic.

DOCUMENTATION REQUIRED

  • Application Form including the Medical Report signed by the applicant’s General Practitioner. 
  • Ticket of Referral provided and filled in by the applicant’s General Practitioner. This referral will be received by a designated key person who will discuss the case with the Community Geriatrician.
