Social Work

The aim of the Social Work service is to provide emotional support, guidance and assistance to senior citizens. This service deals with social casework, provides advocacy for clients, facilitates self-help management and develops action plans, performs crisis intervention work, provides assessments for Home Help Service and liaises with the geriatric and general rehabilitation hospitals.

This service also deals with reports on senior citizens having acute conditions who are residing in residential homes.  The Social Work Unit which manages this service also assess social cases and deals with them accordingly. This Unit also liaises with the Health Department, the police, local councils, parish priests and other community organisations.

WHAT YOU'LL GET

Social work intervention and follow-up of the client to alleviate present and emerging stressful situations. The aim is to empower clients in the self-help process to enable them to build a positive attitude.

When necessary, the assistance of specialised professional agencies is sought to tackle emerging situations more effectively.

ELIGIBILITY

General eligibility criteria:

  • Senior citizens over 60 years of age who are living alone and have a high level of dependency; or
  • Elderly suspected to be suffering from physical, psychological, social or financial abuse; or
  • Elderly suffering from dementia or other cognitive impairment; or
  • Elderly living in a state of distress or unhappiness

DOCUMENTATION REQUIRED

General documentation:

  • Filled application form including the medical report signed by a General Practitioner. 
  • Any relevant documents that can contribute to a better plan of action as necessary
  • Any medical certificate or a hospital discharge report
